It seems the problem is fixed. However I noticed that there are 4 index.dat files that no longer show up in the list of files to be deleted -

Documents and Settings\%username%\Cookies\
Documents and Settings\%username%\UserData\
Documents and Settings\%username%\Application Data\Microsoft\Office\Recent\
Documents and Settings\%username%\Local Settings\Application Data\Microsoft\Feeds Cache\

Should these be deleted, or are they handled differently now in relation to cookie handling?

I prefer Index.dat Suite as it searches for all index.dat files and list them in its main panel.

In the Tools section of Index.dat Suite you can select to run defrag upon reboot which is like booting up in Safe Mode then running defrag.
